INSTRUCTIONS

WARNING: Clicking the RESET button at the bottom of the order form clears all the entries.

  1. Scroll down until you see NAME, click in the white box under NAME to begin entering your information.
  2. You can use your mouse to get to the next catagory by clicking in the appropriate white box or simply use the TAB key on the keyboard to move ahead to the next catagory.
  3. To go back and edit an entry, click in the appropriate box and make your changes.
  4. The order form allows you to enter 20 orders, if you need more, do the following, if not continue to step 5.
  5.  

    a) Finish the first order form, type in the appropriate Number of Pages section (i.e.: Page 1 of 2 for 1st order form) click the SUBMIT button and print out the following page as your 1st order form receipt. After printing the receipt, use the BACK button on your browser to go back to the previous page (Order Form).

    b) If there are entries still on the form, click on the finger pointing down (BOTTOM) on left side of the page to get to the bottom of the form and click the RESET button to clear the entries on the form. Click the finger pointing up (TOP) to return to the top of the form.

    c) Scroll down until you see NAME again, click in the white box under NAME to begin entering your information for your second order form. When your done continue to step 5.

  6. When you are done entering all your orders, scroll down to the bottom of the page and fill-in the required boxes: TOTAL, TAX, GRAND TOTAL, How Many Oders & Number of Pages (i.e.: Page 1 of 1 for one order form: Page 1 of 2 for 1st. form of a 2 form order and Page 2 of 2 for 2nd. form of 2 form order) click the SUBMIT button and print the following page as your receipt for your order, DONE.
